Pennington Dental welcomes all patients, whether you're private, under the NHS, an existing Denplan client or wish to join Denplan. If you'd like to become a patient of Pennington Dental, please complete the form below.

If you have already registered online please return your paperwork to us as soon as possible.  As you can imagine we have had an unprecedented response and all who have returned their forms will be offered appointments as soon as we open. 

We are due to open in early August and will be contacting you soon to arrange your first appointment.
